Sports Participation Information & Forms
Before students can participate in athletic practices they must have the following FOUR documents on file in the school office:
Doctor Physical Form (English : Page 4 Parent Consent - Spanish)
Chiropractor List for Physicals (English)
Concussion and Head Injury Release Form
When needed, to pick up your student from an event, you can use this transport form. (English)
